If you are using POP mail, give POPFile a try.
I've been running it for about a month, and it's running at 99.65% accuracy. It uses a Bayesean filtering system which, IMHO, is the only type of SPAM filter that actually works. It's really a cool program to play with too :) The statistics page is a hoot... After a week of minor training, it's damn near perfect. Out of thousands of emails, I've only had 1 false positive.
